Resurrection House Inc. provides shelter services for single males and appropriate supportive housing for homeless woman and children. We operate a shelter that supports a maximum of eight men in an emergency shelter setting and 13 families in transitional affordable housing. The Program Manager reports to the Executive Director and works closely with the D.S.S. representatives as well as the Resurrection House Inc. staff.

 

Job Summary: 

• Conducts initial intake and orientation with new clients

• Completes Suffolk County Department of Social Services documentation

•  Engages and reinforces the clients in discussions about Residence rules and regulations
  including chores

•  Provides follow up and resolution to all client incident reports and complaint reports as
  required

•  Maintain professional relationships with clients and respects client confidentiality

•  Creates and maintains client files

•  Develops Independent Living Plans with clients and periodic reassessments

•  Conducts intensive case management including collaboration with outside agencies to
  secure wrap around services for clients

•  Responsible for making referrals to necessary resources, advocating for clients and
  monitoring individual’s needs

•  Monitor clients’ progress both Shelter and Transitional Families toward achieving their
  goals via face-to-face meetings or scheduled telephone contact and document via
  progress notes.

•  Provides all required information for weekly/monthly/quarterly/annual reports.

•  Escorts clients to appointments (educational, medical, social service, etc) as needed

•  Enhance individual’s daily living skills

•  Assists in permanent housing search and completing housing packets

•  Completes Discharges

•  Maintains client binders

•  Keeps supervisor informed of all program and client developments

•  Follow-up with clients for a period of time after successful completion of their primary
  goals to assure client stability.

•  Organizes third party informational sessions

•  Supervises Shelter Support Staff and prepares monthly schedule.

•  Schedules Shelter Support Staff to ensure 24/7 coverage

•  Must be available to be on call as required

•  Performs other duties as assigned

Requirements:

• Master’s Degree preferred

• Ability to deal calmly in crisis situations;

• Experience in Human Services and working with a variety of populations including individuals experiencing homelessness.

Salary: $65,000/year
